29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury. On or about September 6, 2022, and at times prior thereto, employees were wiring light fixtures on a 277-v circuit without training in the control of hazardous energy sources, exposing employees to electrical shock.

29 CFR 1926.417(b): Equipment or circuits that were de-energized were not rendered inoperative. On or about September 6, 2022, and at times prior thereto, employees were wiring light fixtures on a de-energized 277-Volt circuit without ensuring the energy source could not be re-energized during the course of work, exposing the employees to an electrical hazard of approximately 277-volts.
