29 CFR  1904.39(a)(2):Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  At 757 Merus Ct, Fenton, MO 63026 - On August 25, 2022 an employee amputated their left pinky finger at a worksite located at 13963 State Hwy AC, Florissant, Mo 63033.  Employer did not report the incident until August 31, 2022.
