29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: On or about September 7, 2022 employees were exposed to fall hazards of approximately 10 -12 feet when accessing the top of the oven used to walk over to the entrance of the catwalks in the Overhead Stain department located in Plant 1. The guardrail did not extend the full length of the unprotected oven on the sides where the fixed ladder is located.

29 CFR 1910.29(b)(1): The top edge height of top rails, or equivalent guardrail system members, were not 42 inches, plus or minus 3 inches, above the walking-working surface: On or about September 7, 2022, the employer did not ensure employees who accessed the catwalk to clear frame jams on line 1 and line 2, located in the Overhead Stain department - Plant 1, were protected from falling. The guardrail system did not have a top rail.
