Failure to provide the required training and evaluation to PIV operators will expose operator, as well as others working in and around forklift operating areas to struck-by injuries, tipping, and rollover incidents.

Employees working in this condition were exposed to potential struck-by injuries from falling rigging equipment, and or falling prefabricated wooden trusses.

Employees installing prefabricated wooden trusses were exposed to falls of 10 ft - 22ft or greater to the ground below.
