29 CFR 1910.178(o)(1):Load(s) on powered industrial truck(s) were not stable or safely arranged: a) For the area near the 515 Machine Center transformer bracket, on or about 08/29/2022, the employer exposed employees to struck-by and/or crushing hazards, in that, a transformer weighing between 1000 and 2000 pounds was not stably and safely set on the forks of powered industrial trucks. Rather, the transformer was set on the forks at an angle, and the forks were too short to extend the width of the transformer while it was supported on the forks.
