29 CFR 1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) This violation occurred on or about September 8, 2022, at a job site located at 6601 Thames Drive, Mesquite, TX, where employees doing decking work on a single-family house where employees were using a Werner 16-feet extension ladder that did not extend 3-feeet above the landing of the roof. Employees were exposed to a fall hazard.

CARLOS FEDERICO VILLATORO WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 1926.501(B) (13) OR ITS EQUIVALENT WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1512949, CITATION NUMBER 2, ITEM 1 THAT WAS ISSUED ON MARCH 12, 2021, WITH RESPECT TO A WORKPLACE LOCATED AT 900SILVERMAPLE, ROYSE CITY, TX 75189. THE FINAL ORDER DATE WAS APRIL 6, 2021.
