29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:  a) Flat Roof on Backside of Building (Yellow Siding) - On or about September 8, 2022, employees accessed a flat roof during ascent and descent for performing roofing work on the main roof of the building with approximately 8:12 pitched roof without any fall protection.  The approximate height from flat roof (Yellow Siding) to the ground was approximately 26 feet in height.  The employees did not have fall protection when working from this flat roof (Yellow Siding) and were exposed to fall hazards of approximately 26 feet.  b) Flat Roof on Backside of Building (Brick Siding) - On or about September 8, 2022, employees accessed a flat roof during ascent and descent for performing roofing work on the main roof of the building with approximately 8:12 pitched roof without any fall protection.  The approximate height from the flat roof (Brick Siding) to the ground was approximately 20 feet in height.  The employees did not have fall protection when working from this flat roof (Brick Siding) and were exposed to fall hazards of approximately 20 feet.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   a) Main Roof - On or about September 8, 2022, employees were performing roofing work on the main roof of the building with approximately 8:12 pitched roof without any fall protection.  The approximate height from ground to eave of the main roof was approximately 30 feet. The employees were not wearing fall protection when working on the main roof and were exposed to fall hazards of approximately 30 feet.

The approximate height from flat roof (Yellow Siding) to the ground was approximately 26 feet in height. 29 CFR  1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided.   a) Flat Roof on Backside of Building (Yellow Siding) to and from the Main Roof - On or about September 8, 2022, employees were not proved a stairway or ladder when accessing either the Flat Roof (Yellow Siding) or the Main Roof of the Building with an approximate climb or descent of 4 feet.

29 CFR  1926.1053(b)(12): Ladders used where the employee or the ladder could contact exposed energized electrical equipment did not have nonconductive siderails:   a) Worksite - On or about September 8, 2022, employees used an electrically conductive aluminum extension ladder with conductive siderails to access and descent from a flat roof (Brick Siding). The approximate height from the flat roof (Brick Siding) to the ground was approximately 20 feet in height.   The aluminum extension ladder was positioned against the building approximately 4 feet away from electrical power lines exposing employees to electrical hazards.
