29 CFR  1910.37(a)(3):Exit routes were not kept free and unobstructed:  On or about September 7, 2022 and times prior, the back emergency exit routes in Dollar Tree Store #1959 located at 2306 S. Jefferson Ave.  Suite M Mount Pleasant, TX were partially obstructed with stored merchandise and/or materials, exposing Dollar Tree employees to being trapped in the event of a emergency requiring evacuation in the following locations;  a) In the back of the store, the office hallway leading to the single-door emergency exit was obstructed with storage racks stacked along the wall restricting the exit route.  b) In the back of the store by the storage room, the path leading to the double-door emergency exit was obstructed by stored merchandise restricting the exit route.  Dollar Tree Stores, Inc. was previously cited for a  violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.37(a)(3), which was contained in OSHA inspection number 1443653, citation number 1, item number 2, and was affirmed as a final order on April 19, 2021, with respect to a workplace located at 2708 Renaissance Dr., Fort Worth, TX 76105.  Dollar Tree Stores, Inc. was previously cited for a  violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.37(a)(3), which was contained in OSHA inspection number 1438186, citation number 2, item number 2, and was affirmed as a final order on April 16, 2021, with respect to a workplace located at 4428 North Fwy, Houston, TX 77022.  Dollar Tree Stores, Inc. was previously cited for a serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.37(a)(3), which was contained in OSHA inspection number 1399905, citation number 1, item number 2, and was affirmed as a final order on April 19, 2021, with respect to a workplace located at 1914 South Main St., Weatherford, TX 76086.  Dollar Tree Stores, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.37(a)(3), which was contained in OSHA inspection number 1293367, citation number 1, item number 1, and was affirmed as a final order on August 20, 2018, with respect to a workplace located at 9208 East RL Thornton, Dallas, TX 75228.

29 CFR  1910.22(a)(1):Passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition.  On or about and times prior-to September 7, 2022 Dollar Tree Store #1959 located suite M of 2306 S. Jefferson Ave. Mount Pleasant, TX storerooms was not kept in a clean and orderly condition based on the following observations:  a) At the back of the store location the office hallway was not kept clean and orderly where mop buckets and other materials were placed along the wall creating a trip hazard.     b) At the back of the store in the storage room walking working surfaces were not kept orderly where boxes of merchandise and collapsed stacks of boxes created a trip hazards.  Dollar Tree Stores, Inc. was previously cited for a triple-repeat serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.22(a)(1), which was contained in OSHA inspection number 1501569, citation number 2, item number 1 and was affirmed as a final order on May 3, 2021, with respect to a workplace located at 1707 East Houston St, Cleveland, TX 77327.  Dollar Tree Stores, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.22(a)(1), which was contained in OSHA inspection number 1443653, citation number 1, item number 1, group a, and was affirmed as a final order on April 19, 2021, with respect to a workplace located at 2708 Renaissance Dr., Fort Worth, TX 76105. Boxes  stored in tiers were not stacked, blocked, interlocked and limited in height so that they were stable and secure against sliding or collapse.  a) On or about September 7, 2022 and times prior-to stacked boxes of merchandise in the back store room reached over 9 feet in height exposing employees to struck by falling object hazards.    b) On or about September 7, 2022 and times prior-to stacked boxes of merchandise in the back store room were not blocked, or interlocked exposing employees to struck by falling object hazards.
