29 CFR 1910.176(a): Permanent aisles or passageways were not appropriately marked, and Aisle(s) and passageway(s) were not kept clear with no obstruction across or in aisles that could create a hazard: a. At the facility's Warehouse (Zone 1) located at 14610 Breakers Drive in Jacksonville, Florida 32258: On or about September 8, 2022, the employer exposed employees to struck-by and crushed-by hazards, in that permanent aisles or passageways used by mechanical equipment, such as powered industrial trucks (forklift and high-reach trucks), were not appropriately marked to keep the equipment separated from employees who worked and walked in the area during mechanical equipment operations. b. At the facility's Warehouse (Zone 1) located at 14610 Breakers Drive in Jacksonville, Florida 32258: On or about September 8, 2022, the employer exposed employees who operated mechanical equipment, such as powered industrial trucks (forklift and high-reach trucks) to struck-by and crushed-by hazards, in that where mechanical equipment was used, there was not sufficient safe clearances that allowed for aisles, at loading docks, through doorways or wherever turns or passage had to be made, during mechanical equipment operations.
