29 CFR 1926.501(b)(2): 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: On or about September 8, 2022, on the west side of Grace International Church in Spring, TX, an employee was engaged in framing a stucco wall at the leading edge with unprotected edges without a personal fall arrest system.

29 CFR 1926.602(c)(1)(ii):Modifications or additions affecting the safe operation of the equipment were made without the manufacturer's written approval: Note: If such modifications or changes were made, the capacity, operation, and maintenance instruction plates, tags, or decals should have been changed accordingly. On or about September 9, 2022, on the west side of Grace International Church in Spring, TX, the employer modified the JLG Skytrak Telehandler, Model 10054 with a personal lift basket without the manufacturers written approval.
