On or about September 08, 2022, at the address of 305 S. Jefferson St. Pandora, OH; The employer failed to ensure the sweeper and floor auger located in bin 90 were disconnected and/or isolated from their power source(s) prior to employees accessing the bin. Three employees performing cleaning tasks inside grain bin in this condition were exposed to amputation hazards.

On or about September 08, 2022, at the address of 305 S. Jefferson St. Pandora, OH; The employer failed to ensure an observer, equipped to aid, was stationed outside bin number 90 as employees occupied the space for cleaning tasks. Without the benefit of an observer position outside the confined space, the three employees performing these tasks inside of the grain bin 90 were exposed to deficiencies in emergency response.
