29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a) Rear Porch Roof: On or about August 9, 2022, an employee was exposed to falls of approximately ten feet from the lower level while replacing shingles and underlayment on a porch roof due to a lack of any guardrail system, safety net system, personal fall arrest system, or monitoring system.  Abatement certification and documentation required.

29 CFR  1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support:  a) Rear porch steps: On or about August 9, 2022, an employee was exposed to a fall hazard due to a ladder not being extended more than three (3) feet above the edge of the roof which was being accessed, nor was it secured at its top.  Abatement certification and documentation required.

29 CFR  1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways were not secured to prevent accidental displacement, or barricaded to keep the activities or traffic away from the ladder:  (a) Rear porch entryway: On August 9, 2022,  a ladder was placed on the steps of the rear porch, in front of a door, exposing employees to a fall hazard  from the ladder not being secured and/or barricaded.  Abatement certification and documentation required.
