1910.147(c)(4)(ii)  The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: 29 CFR  1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.   On or about September 2, 2022, the employer did not develop an energy control procedure, for the Marq Carton erector machine number 6, which included specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy, specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them, and specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.  Employees were exposed to hazardous energy which resulted in an injury.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions listed and shall be done in sequence as required by 29 CFR 1910.147(d)(1)-(6):   (1) prepare for shutdown, (2) shut down machine or equipment, (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (4) affix lockout or tagout devices to each energy isolating device by authorized employee, (5) relieve, disconnect, restrain or otherwise render safe all stored or residual energy, (6) verify that isolation and deenergization of the machine or equipment have been accomplished:  On or about, September 2, 2022 an employee failed to lock out carton erector number 6 while unjamming a box and came into contact with moving parts. The machine was not powered down and locked out, following the sequence above, resulting in an amputation.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.  On or about September 8, 2022 and before, the employer did not provide a method of guarding on the Carton erector machine number 6. Employees were exposed to unguarded chains and sprockets on the hopper, stack lift, and the exit portion of the machine.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
