29 CFR  1910.133(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about September 22, 2022, with respect to an establishment located at 544 Acme Street, Green Bay, WI 54302, an employee was observed using PC-622 Peroxyacetic Acid cleaner, and had earlier dispensed the chemical, without using eye protection.

29 CFR  1910.138(a): Employers shall select and require employees to use appropriate hand protection when employees' hands are exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes.  On or about September 22, 2022, with respect to an establishment located at 544 Acme Street, Green Bay, WI 54302, an employee was observed using PC-622 Peroxyacetic Acid cleaner, and had earlier dispensed the chemical, without using hand protection capable of preventing chemical burns.

29 CFR  1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  On or about September 22, 2022, with respect to an establishment located at 544 Acme Street, Green Bay, WI 54302, employees dispensing PC-622 Peroxyacetic Acid cleaner into secondary containers from a fall mounted spigot in the Fresh trim area where not protected from serious eye injuries by the presence of an eye wash station capable of providing 15 minutes of emergency eye flushing.

29 CFR  1910.1200(f)(6): Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with the information identified in paragraphs 1910.1200(f)(6)(i) or 1910.1200(f)(6)(ii).  On or about September 22, 2022, with respect to an establishment located at 544 Acme Street, Green Bay, WI 54302, an employee was observed using a 2 gallon spray tank that was labeled as containing KC-644 Anti-Microbial solution but was discovered to contain PC-622 Peroxyacetic Acid cleaner. This exposed employees to potential chemical hazards related to the mis-identification of the product in the container.

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  On or about September 22, 2022, with respect to an establishment located at 544 Acme Street, Green Bay, WI 54302, employees operating the combo dumper in the Fabrication Department were exposed to potential caught-in hazards when the dumper were not guarded to prevent access to a pinch point created by the dumper and frame above the transfer conveyor.

29 CFR  1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  On or about August 25, 2022, with respect to an establishment located at 544 Acme Street, Green Bay, WI 54302, employees operating the Side Puller in the Harvest Department were exposed to potential caught-in hazard when the activation buttons for the clamping mechanism were located in an area that allowed access to the point of operation.
