29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   a) Building Exterior - Roof: Employees were exposed to eye and face injuries while doing stucco on the roof area without the use of eye and face protection, on or about 9/7/22.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.    a) Building Exterior Back: Employees were using pipe scaffold frames and cross braces to access the scaffold, exposing employees to a fall of approximately 14 feet to the ground surface, on or about 9/7/22.

29 CFR  1926.1051(a):  Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:  a) Roof area: The employer did not provide a ladder or other safe means of access for employees accessing the roof area. Employees were using the scaffold frames to access and transition from the scaffold to the roof area where work was taking place, on or about 9/7/22.
