Alpine Road, Machesney Park, IL 61115  On or about September 1, 2022, Curtis Metal Finishing Company, did not develop and implement an emergency response plan prior to employee(s) proceeding to conduct emergency response operations for a chemical spill (Ultrax 94D) on Line 77 Feeding Station.  Employees were exposed to chemical burns.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Alpine Road, Machesney Park, IL 61115  On or about September 1, 2022, Curtis Metal Finishing Company, did not assess the workplace to determine if hazards were present, to ensure that employee(s) have the adequate Personal Protective Equipment (PPE) such as, but not limited to, goggles, a chemical resistant apron, rubber nitrile boots, or other PPE needed to conduct emergency response operations for  a chemical spill of UltraxD94, at Line 77 Feeding Station.  Employees were exposed to chemical burn hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Alpine Road, Machesney Park, IL 61115  On or about September 1, 2022, employee(s) used and werer exposed to hazardous chemicals such as, but not limited to, Ultrax 94D, and other solvents.  The employer did not develop or implement a written hazard communication program that includes the above mentioned elements.  Employees are exposed to chemical burn hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
