29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) This violation occurred on or about September 10, 2022, at a job site located at 390 TX 557 Spur, Terrell, TX; where employees were installing glass windows to a new build commercial Cadillac Platinum building while working and walking around on the ground at ground level and were exposed to head injuries where overhead hazard exists by not wearing hard hats.

29 CFR 1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: This violation occurred on or about September 10, 2022, at a job site located at 390 TX 557 Spur, Terrell, TX, where employees were installing glass windows to a new build commercial Cadillac Platinum building while using a Genie lift. Employees did not have on a body belt or safety harness and lanyard and was exposed to a fall hazard. (a) At least one (1) employee inside the basket of the Genie lift, S-40, serial number 10196941 at the right side of the lot with the with the white liner paper. The employee was exposed to fall hazard of approximately 33-feet (b) At least two (2) employees inside the basket of the Genie lift, S-65XC, serial number 65849 at the left side of the lot with the blue liner paper. The employee was exposed to fall hazard of approximately 20-feet.
