29 CFR 1910.334(a)(2)(i): Portable cord- and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence for possible internal damage (such as pinched or crushed outer jacket).  Storage Building: The orange extension cord was plugged in and energized. The plug end of the cord was damaged and the inner screws were exposed. The cord was not visually inspected as required. The extension cord was being used to power a Makita LS 1011 portable miter saw. Employees were exposed to electrical hazards.
