29 CFR 1926.453(a)(2): Field modification of the aerial lift was not certified in writing by the manufacturer or by any other equivalent entity, such as a nationally recognized testing laboratory, to be in conformity with all applicable provisions of ANSI A92.2-1969 and this section and to be at least as safe as the equipment was before modification: location: exterior of building, above sidewalk on or about: 9/9/22 a) An employee was working from a truck mounted aerial lift. The midrail chain and the hardware for the connection of the midrail chain, in the basket entry area, were missing. The employee was exposed to a fall of 14 feet. NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.

The employee was not wearing a body belt nor was a lanyard attached to the boom or basket. The employee was exposed to a fall of 14 feet. NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.
