29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures. a) On or about August 10, 2022, throughout the jobsite five employees performing residential framing activities were observed walking through discarded materials with protruding nails.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about August 10, 2022, during residential framing activities, employees were exposed to a greater than 6-foot fall while securing Tyvek house wrap on a roof without using fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. a) On or about August 10, 2022, during residential framing activities, employees were exposed to a greater than 6-foot fall while using an extension ladder that was not set three feet above the roof landing.
