29 CFR  1926.502(d)(15):Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached.  a. On or about September 12, 2022, at a worksite located at 5400 Byers Circle East, Columbus Ohio, 43229 the employer failed to ensure independent anchorage equipment that was able to support at least 5,000 pounds was available to the employees. The employees were attached to lifelines that were wrapped around 2 x 4 trusses thereby exposing the employees to a 17-foot fall hazard.

29 CFR  1926.20(b)(1):It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.  a. On or about September 12, 2022, at a worksite located at 5400 Byers Circle East, Columbus Ohio, 43229 the employer failed to maintain a safety and health program to address hazards such as fall protection during framing activities.  Jose Martinez Drywall, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.20(b)(1), which was contained in OSHA inspection number 1438335, citation number 1, item number 1 and was affirmed as a final order on March 18, 2020, with respect to a workplace located at 700 West Waterloo, CANAL WINCHESTER, OH, 43110.

On or about September 12, 2022, at a worksite located at 5400 Byers Circle East, Columbus Ohio, 43229 the employer failed to train each employee in the recognition of unsafe conditions and the regulations required to control or eliminate the hazards. Employees were setting trusses and didn't know or understand the requirements associated with the use of fall protection and mobile supported scaffold work platforms.  Jose Martinez Drywall, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.21(b)(2), which was contained in OSHA inspection number 1438335, citation number 1, item number 2 and was affirmed as a final order on March 18, 2020, with respect to a workplace located at 700 West Waterloo, CANAL WINCHESTER, OH, 43110.
