29 CFR 1904.39(a)(2):The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. 410 Sackett Point Road, Building #15, North Haven, CT: On or about September 6, 2022, an employee fell off a ladder while inspecting an apparent water leak above ceiling tiles and sustained a broken hip. The employee was admitted to Yale New Haven Hospital on the same day. The in-patient hospitalization was not reported to OSHA until September 12, 2022.
