29 CFR  1910.147(c)(1):Energy control program. The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was not isolated from the energy source and rendered inoperative.  At 1605 Park 370 Place, Hazelwood, Mo 63042 - On September 14, 2022 employees of Agile Packaging Solutions were exposed to hazards associated with the unexpected energization of equipment due to not having a lock out tag out program during servicing and maintenance of machines and equipment.   Employer has no lock out / tag out program when servicing of an MPAC Switchback AC-6H s/n 1738, Quickflex 400 s/n QF 400-001 and various other packaging machines and conveyers.

29 CFR  1903.2(a)(1):Each employer did not post and keep posted a notice or notices, to be furnished by the Occupational Safety and Health Administration, U.S. Department of Labor, informing employees of the protections and obligations provided for in the Act, and that for assistance and information, including copies of the Act and of specific safety and health standards, employees should contact the employer or the nearest office of the Department of Labor. Such notice or notices were not posted by the employer in each establishment in a conspicuous place or places where notices to employees are customarily posted.
