29 CFR  1926.102(a)(1):  The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    a) At the site, where the employer did not ensure that employees used eye or face protection while applying Holcim cement to the concrete masonry units.  Holcim cement is a corrosive and contains Portland cement and silica.  Caceres Masonry, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1552390, citation number 1 , item number 2, and was affirmed as a final order on May 23, 2022, with respect to a workplace located at 11665 Highway 6 N, Sugar Land, TX 77498.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:    a) At the site on September 14, 2022, where the employer did not ensure employees working from a two tiered scaffold greater than ten feet in height had a way to access the platform on the northeast side of the building.  Employees climbed up the side of the scaffold to access the platform.  b)  At the site on September 14, 2022, where the employer did not ensure employees working from a two tiered scaffold greater than ten feet in height had a way to access the platform on the southeast side of the building.  Employees climbed up the side of the scaffold to access the platform.
