29 CFR  1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a.  On or about September 14, 2022, at the boiler room, the employer exposed employees to a 10 feet fall hazard, in that, operators were required to reach hand-valves located on top of unguarded boiler tanks.

29 CFR  1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems:  a.  On or about September 14, ,2022, at the northwest area behind the boiler room, the employer exposed employees to a fall hazard, in that, a temporary open hole measuring 28" deep x 34" W x 60" L did not have a protective cover or an affective guarding system.    b.  On or about September 14, ,2022, at the DA tank area in the 2nd floor, the employer exposed employees to a trip and fall hazard, in that, a 10 inch square floor drainage was missing a cover.

29 CFR  1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by approved cabinets or other forms of approved enclosures, or other means listed under this provision:  a.  On or about October 18, 2022, behind the diesel pumps, the employer exposed employees to electrocution  hazards, in that, an electric cable had a live exposed conductor that was not properly guarded with a cover or de-energized.

On or about October 18, 2022, at the front of the diesel pumps, the employer exposed employees to electric shock hazard, in that, a junction box with live conductors was missing a protective cover.
