29 CFR  1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided:  a) Third floor roof/1521 Tyron Rd, NE, Atlanta, GA - On or about September 14, 2022, the employer exposed employees to struck by hazards in that, the employer failed to ensure that personal protection equipment was provided to employees  when they were installing shingles on the third floor roof, approximately 35 feet above the ground while other employees were working with shingle hoist/ladder platform on the ground.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems or personal fall arrest systems:   a)Third floor roof/1521 Tyron Rd, NE, Atlanta, GA - On or about September 14, 2022, the employer exposed employees to fall hazards of approximately 35 feet, in that the employees were not provided fall protection devices when they were on the third floor roof installing shingles.
