29 CFR 1910.22(d)(1): Walking-working surfaces were not inspected, regular and as necessary, and maintained in a safe condition: On or about September 15, 2022, employees were exposed to trip and fall hazards when walking along areas with broken floor tiles: a. At the reception area. A 2-inches in diameter section of vinyl floor tile deteriorated, creating a depression, and was not replaced. b. At the Clinical Pharmacist Supervisor's office. Vinyl floor tiles underneath the desk popped up, creating a sharp bulge and were not replaced. c. At ADPAC's office area. The vinyl floor tile along the right side (9-inches) of the door frame was broken and was not replaced. NOTE: Certification of abatement is not required for this citation item since it was submitted prior to notice issuance.
