29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Shop Floor: On or about 9/15/22, an employee sustained an injury at the point of operation after the operator cycled the T15 Pneumatic Toggle Press, model number RMT #3A, which was not provided with adequate machine guarding methods to protect employees from pinch-points. The press was not provided with point of operation guarding and the hand controls were subject to activation from electrical interference from mobile phones. (b) Shop Floor: On or about 10/18/22, the T9 Pneumatic Press was not provided with adequate machine guarding methods to protect employees from pinch-points in that the press was not provided with adequate point of operation guarding.

29 CFR 1910.217(c)(5)(i): Mechanical power press(es) where the operator feeds or removes parts by placing one or both hands in the point of operation, and a two hand control, presence sensing device, or type B gate or movable barrier guard is used, did not have a control system and a brake monitor: a) Shop floor: On or about 10/18/22, the employer did not ensure that mechanical power presses, including the T7 press, were provided with control reliability and a brake monitor to prevent a successive stroke while hand-in die feeding parts for production.
