Multi-plastics, Inc., Interior: On or before 9/15/2022, and times prior, the employer exposed employees to safety hazards, in that the employer failed to conduct a hazard assessment of the workplace where employees perform work practice such as: cutting with slitter machines, grinding of metal parts, and forklift operations.

29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a). Production area, interior: On or about September 15, 2022 and times prior, the employer exposed employees to amputation hazards, in that guarding is not provided on slitter machines while employees are working next to machines. CSHO observed employees working next to slitter machines in operation and employees standing in danger zone.

29 CFR 1910.178(l)(6):Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. a). Production area, interior: On or about 9/8/2022, and at times prior, employees are exposed to struck-by hazards, in that the employer required employees to operate powered industrial forklift trucks without certification, evaluation, and training on the type of truck they were operating.
