29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On or about September 07, 2022, the employer did not ensure that the Slider Rail Straightener had adequate guarding to prevent the operator from being exposed to amputation and/or caught-in hazards during normal operations. a) The infeed end of the Slider Rail Straightener was not guarded to prevent the operator of the from having their hand and/or fingers in the danger zone during the in-feeding of roll stock. b) The series of roll formers were not guarded to prevent employees from making contact with the danger zone.
