29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) Single family home roof structure, various locations; Workers engaged in roof replacement tasks from a 3:12 pitch roof were not protected from falls while working at heights ranging between approximately 18-feet, 10-inches to 22-feet, 2-inches above ground level.   (b) Garage roof structure, various locations; Workers engaged in roof replacement tasks from a 5:12 pitch roof were not protected from falls while working at heights ranging between approximately 8-feet, 11-inches to 16-feet above ground level.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a) Single family home roof structure, various locations; Workers exposed to fall hazards at heights ranging between approximately 18-feet, 10-inches to 22-feet, 2-inches above ground level were not trained on fall hazard recognition and mitigation methods.   (b) Garage roof structure, various locations; Workers exposed to fall hazards at heights ranging between approximately 8-feet, 11-inches to 16-feet above ground level were not trained on fall hazard recognition and mitigation methods.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) East end of single family home, ground level; The side rails of a 32-foot Werner brand model no. D1332-2 Type-1 aluminum extension ladder used by employees to access and egress the roof did not extend at least 3 feet above the landing surface.
