The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: (a) Residential structure, upper roof; Workers engaged in roofing construction activities from a steep-slope roof at heights up to approximately 24-feet, 4-inches above ground level were attached to rope lifelines that were connected to the front of climbing-type body belts at waist-level.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer: (a) Residential structure, upper and lower roof; The employer did not prepare a written record to certify that workers performing roofing construction activities at heights up to approximately 24-feet, 4-inches above ground level, received training on applicable topics, including but not limited to fall hazard recognition and mitigation procedures. Quality Roofing & Remodeling, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.503(b)(1), which was contained in OSHA inspection number 1445000, citation number 2, item number 1 and was affirmed as a final order on April 2, 2020, with respect to a workplace located at S520 Church Avenue, Spring Valley, WI 54767.
