29 CFR 1910.176(a): Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked. a) On or before 9/15/2022 in the stacking area and the pallet room, the employer did not ensure that the aisles utilized by the clamp trucks to stack/unstack paper rolls maintained a sufficient safe clearance in that employees were exposed to struck-by hazards due to rolls being stack as high as 16' in aisles where powered industrial vehicles operated.

29 CFR 1910.176(b):Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. a) The employer did not ensure that stacked rolls of paper were properly stacked in order to prevent tip overs, employees were exposed to struck by hazards while stacking/unstacking the rolls of paper utilizing Hyster forklifts. On 9/7/2022, in the maintenance department a Hyster clamp truck was left with the roll cage damaged due to a roll of paper being dropped on the roll cage.

29 CFR 1910.178(o)(1):Only stable or safely arranged loads shall be handled. Caution shall be exercised when handling off-center loads which cannot be centered. a) On 8/29/2022 an employee was in the process of stacking two 36" rolls on top of stack of paper rolls, the employee noticed that they were not close enough to the paper stack, with the clamped paper rolls extended all the way up (approx. 15'-7") and 8-10" inches out, as the employee began to reverse the Hyster forklift it tipped forward and came to rest on its side on the clamped paper rolls.
