29 CFR 1910.119(d): Process safety information. In accordance with the schedule set forth in paragraph (e)(1) of this section, the employer shall complete a compilation of written process safety information before conducting any process hazard analysis required by the standard. The compilation of written process safety information is to enable the employer and the employees involved in operating the process to identify and understand the hazards posed by those processes involving highly hazardous chemicals. This process safety information shall include information pertaining to the hazards of the highly hazardous chemicals used or produced by the process, information pertaining to the technology of the process, and information pertaining to the equipment in the process. On, or about, September 19, 2022, the employer had not included the manufacturing operations carried out at LYONDELL-BASELL (DBA Lake Charles Polyotetins Plant) K line in the company's Process Safety Management program. Process safety information pertaining to the hazards of the highly hazardous chemicals in the process such as lower explosive/flammable limits, permissible exposure limits, physical data, reactivity data, and stability data, and the hazardous effects of residual "heptane" fumes and the static electrical output of vacuuming desiccant per Process Safety Information.

29 CFR 1910.119(h)(3)(i):29 CFR 1910.119(h)(3)(I): The contract employer did not assure that each contract employee is trained in the work practices necessary to safely perform his/her job. On or about September 14, 2022, employees were exposed to fire, explosion, and toxic gas release hazards when the employer did not develop and implement safe work practices for opening process equipment (T-72503B) to remove/replace desiccant and ensure the Lower Explosive Levels (LEL) of the flammable hydrocarbons. a) Supervisor was not trained b) Foreman was not trained c) Laborer was not trained
