29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: Site- On or about September 14, 2022 employees were not protected from fire and explosion hazards where the employer was using an excavator to expose a buried 4 inch HDPE natural gas line, 36 inches below grade. The excavator contacted contacted the buried gas line and broke the gas line, creating an uncontrolled release of natural gas into the atmosphere.
