OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were not protected from the hazards associated with the unintentional movement of medical transport vehicles due to not engaging the parking brakes. a) 235 West Cheltenham Avenue, Philadelphia, PA 19144, Street Area - The employer did not ensure employees who operate medical transport vehicles such as Ford Transit 2019 250 series, etc. to transport patients leave the vehicle without first engaging the parking breaks and securing the vehicles from unintentional movement, exposing them to struck by hazards. Condition noted on or about September 10, 2022

The employer reported the fatality on September 12, 2022, more than 8 hours after they became aware of the death.
