Location: Trucchi's Supermarket, 2941 Acushnet Ave., New Bedford, MA 02745 On or about September 12, 2022, an employee was exposed to fall hazards of more than 20 feet while working on the roof from within an aerial lift, where a lanyard was not attached to the boom or basket.

29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Location: Trucchi's Supermarket, 2941 Acushnet Ave., New Bedford, MA 02745 On or about September 12, 2022, employees were exposed to electrical hazards while utilizing an extension cord where the insulation was damaged.
