29 CFR  1926.20(b)(1):It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.  a) Alberto Soto located at 136 Natalie, Lot 333EL, Granville, Ohio 43023: On and preceding September 9, 2022, the employer did not develop and maintain an effective safety and health program to ensure that hazards were identified and corrected onsite while employees performed framing work which exposed them to fall and electrical contact hazards.

The path to ground from circuits, equipment, and enclosures shall be permanent and continuous.  a) Alberto Soto located at 136 Natalie, Lot 333EL, Granville, Ohio 43023: On and preceding September 9, 2022, the employer did not ensure employees used extension cords that had a continuous path to ground. Employees used a yellow extension cord was missing the ground prong, thereby exposing employees to 119-volt electrical contact hazards.  b) Alberto Soto located at 136 Natalie, Lot 333EL, Granville, Ohio 43023: On and preceding September 9, 2022, the employer did not ensure employees used extension cords that had a continuous path to ground. Employees used a orange extension cord where the power cord was missing the ground prong, thereby exposing employees to 119-volt electrical contact hazards.  c) Alberto Soto located at 136 Natalie, Lot 333EL, Granville, Ohio 43023: On and preceding September 9, 2022, the employer did not ensure employees used extension cords that had a continuous path to ground. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.  a) Alberto Soto located at 136 Natalie, Lot 333EL, Granville, Ohio 43023: On and preceding September 9, 2022, the employer did not ensure strain relief was provided to cords so that the pull was not directly transmitted to joints or terminal screws. Employees used a circular saw where the jacket of the cord separated from the strain relief on both ends of the cord, thereby exposing employees to a 119-volt electrical contact hazard.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  a) Alberto Soto located at 136 Natalie, Lot 333EL, Granville, Ohio 43023: On and preceding September 9, 2022, the employer did not ensure that employees engaged in residential construction activities 6 feet or more above lower level were protected from falls. A framer was working on the roof of a residential home at heights up to 12 feet. The employee was not utilizing any form of fall protection, thereby being exposed to falls hazards of 12 feet.

29 CFR  1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  a) Alberto Soto located at 136 Natalie, Lot 333EL, Granville, Ohio 43023: On and preceding September 9, 2022, the employer did not ensure ladders used for the access to landing surfaces were extended at least 3 feet above the landing surface. Employees used an extension ladder where the ladder did not extend at least 3 feet above the edge of the edge of the roof, thereby exposing employees to a 12-foot fall hazard.
