29 CFR 1926.502(i)(4): All covers were not color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard: a) On or about September 15, 2022, at the above addressed jobsite, the employer did not ensure that all floor hole covers were properly marked, thereby exposing employees and subcontract employees to hazards associated with falls of approximately nine (9) feet to the floor below.
