29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) On the east side of a two-level twin home roof; During residential roof sheathing operations, employees were exposed to falls ranging from 17' 9" to 25' 6" while walking on a partially sheathed roof without a fall protection system in place. (b) On the north side second floor wall top plate; During residential truss bracing operations, employees were exposed to falls ranging from 8' 2" to 17' 9" while standing on the top plate without fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Residential roof and trusses; Workers exposed to falling from distances of 8' to 25'6" to lower levels were not trained on the recognition of fall hazards and mitigation methods.
