29 CFR  1926.501(b)(2)(i):Each employee who was constructing a leading edge 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:  a) 14098 E Iliff Ave, AURORA, CO 80014: On or about, Sept 16, 2022, and at times prior, employees were exposed to fall hazards while working on the leading edge of a commercial roof with a fall distance of 20 feet 2 inches to the ground below, without fall protection.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) 14098 E Iliff Ave, AURORA, CO 80014: On or about, Sept 16, 2022, and at times prior, employees were exposed to fall hazards while using a Husky Step Ladder to access and egress the roof. The Husky Step Ladder did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.
