Defective rigging equipment was not removed from service. On or about September 15, 2022, the employer did not ensure the wire rope sling was inspected prior to use and employees were in use of a wire rope sling that was defective. This exposed the employees to a struck-by hazard.

Employees were not required to stand away from any vehicle being loaded or unloaded to avoid being struck by any spillage or falling materials. On or about September 15, 2022, an employee was working next to and under a storm drain being hoisted by digging equipment. This exposed the employee to struck-by and caught-in hazards.
