29 CFR 1926.405(b)(2): All pull boxes, junction boxes, and fittings were not provided with covers: a) At the hotel renovation jobsite located at Spirit Mountain Casino, 27100 Salmon River Hwy, Grand Ronde OR, on 09-16-2022, a pull/junction box cover was found missing, leaving the energized conductors in insulation exposed to contact. The employer failed to ensure that all junction/pull-box with live conductors had covers.

29 CFR 1926.405(j)(1)(i): Fixtures, lampholders, lamps, rosettes, or receptacles had live parts normally exposed to employee contact: a) At the hotel renovation jobsite located at Spirit Mountain Casino, 27100 Salmon River Hwy, Grand Ronde OR, on 09-16-2022, receptacles faceplates were found missing, leaving the energized terminal screws exposed to contact. The employer failed to ensure that receptacles less than 8 feet high have all energized parts of protected by faceplates to prevent employee's contact.
