1926.502(h)(1)(v) The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: a) On the site located at 4801 W Leona St, Tampa FL: On or about 9/17/2022 the employer exposed employees to fall hazards, in that fall protection was not provided to employees while they were conducting roofing activities on a 4:12 pitch roof without the use of fall protection.

1926.1060(a)(1)(iii) The employer did not ensure that each employee using ladders and stairways had been trained by a competent person in the proper construction, use, placement, and care in handling of all stairways and ladders: a.) At a residence, located at 4801 W Leona St, Tampa FL: On or about 9/17/2022 the employer exposed employees to fall hazards, in that the ladder in use to access the first roof from the ground did not have side rails extending at least 3 feet (.9 m) above the landing surface, the railings were approximately 2 feet above the roof edge. b.) At a residence, located at 4801 W Leona St, Tampa FL: On or about 9/17/2022 the employer exposed employees to fall hazards, in that the ladder in use from the first roof to the second roof landing did not have side rails extending at least 3 feet (.9 m) above the second roof landing surface, the railings were approximately 1 foot above the roof edge.
