29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a) 6757 U.S 49 jobsite - On or about September 15, 2022 employees were exposed to a fall hazard while working on the edge of the roof without fall protection approximately 12 feet high.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridgid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the the ladder:   29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to rigid support:  (a)  6757 U.S. 49, Hattiesburg MS - On or about September 15, 2022, employees were exposed to a fall hazard while climbing the 12- 24 ft ladder to access the roof that was not secured to the building.   (b)  6757 U.S. 49, Hattiesburg MS - On or about September 15, 2022, employees were exposed to a fall hazard while climbing the 12 -24 ft ladder to access the roof that did not extend 3 feet above the landing.
