29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of job sites, materials, and equipment to be made by a competent person a) 1 Owl Court, Evesham Township, NJ: Employees were exposed to fall hazards of approximately 20 feet to the ground below when they were not utilizing personal fall arrest systems while installing a new residential roof. The condition was observed on or about September 16, 2022. The Sequeira Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 19260501 B13, which was contained in OSHA inspection number 1535171, citation number 1, item number 1 and was affirmed as a final order on 03/01/2022, with respect to a workplace located at 100 Heather Dr. Mount Laurel, NJ 08054. The Sequeira Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 19260501 B13, which was contained in OSHA inspection number 1389524, citation number 1, item number 1 and was affirmed as a final order on 07/12/2019, with respect to a workplace located at 303 Pleasant Valley Ave., Moorestown, NJ 08057.

29 CFR 1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support. a) 1 Owl Court, Evesham Township, NJ (North): Employees were exposed to fall hazards when a Werner Portable ladder was not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access to a residential roof.
