29 CFR 1926.501(b)(4):Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) Jobsite: 2400 -0 2418 Huntingdon St. Philadelphia, PA 19125 - On or about September 9, 2022, the employer did not ensure that employees conducting framing activities in proximity to an open floor hole were protected from falling through the hole by the use of personal fall arrest systems, a cover, or guardrail system erected around the floor hole. The hole measured approximately 2 feet by 5 feet with an 11 foot fall distance to the lower level.
