29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:                                                                                                             a) Exterior, ground level: An employee was exposed to eye/face injuries while using a 4 inch grinder and were not protected with any type of eye/face protection. Th employee was cutting a sheet of metal roof, on or about 9/14/22.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.  a) Building Exterior: Employees were exposed to a 22 foot fall hazard while engaged in roofing activities from a pump jack  scaffold that was 28 feet long. The end of the platform was not designed and installed so that the cantilevered portion of the platform was able to support employees without tipping, the pump jack scaffold had an 4 foot overhang over the support, on or about 9/14/22
