29 CFR  1910.106(d)(4)(i):  Inside storage rooms shall be constructed to meet the required fire-resistive rating for their use. Such construction shall comply with the test specifications set forth in Standard Methods of Fire Tests of Building Construction and Materials, NFPA 251-1969. Openings to other rooms or buildings shall be provided with noncombustible liquid-tight raised sills or ramps at least 4 inches in height, or the floor in the storage area shall be at least 4 inches below the surrounding floor. A permissible alternate to the sill or ramp is an open-grated trench inside of the room which drains to a safe location:  At a facility located at 950 Breezewood Lane in Neenah, WI; the ink storage room inside the facility contained 55-gallon drums of flammable liquids and was not equipped with a liquid tight raised sill, ramp or open-grated trench inside the room at a doorway opening. A trench was present around the doorway opening; however, the trench was covered with diamond plate steel which would not allow for the flow of flammable liquids into the trench.

29 CFR  1910.147(c)(4)(ii):  The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: (A) A specific statement of the intended use of the procedure; (B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and (D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures:  At a facility located at 950 Breezewood Lane in Neenah, WI; the employer had developed an energy control procedure for Press 1, identified as the Mark Andy P7 Printing Press (Model P7-17, S#1501699/2014). The energy control procedure did not include a specific statement of the intended use of the procedure for Operators nor did it include specific requirements for testing a machine/equipment to verify the effectiveness of lockout devices.

29 CFR  1910.147(d):  The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in sequence as required by 29 CFR 1910.147(d)(1)-(6):   On or about August 30, 2022, at a facility located at 950 Breezewood Lane in Neenah, WI; employees were exposed to machine hazards associated with moving parts when they engaged in cleaning activities on Press 1, identified as the Mark Andy P7 Printing Press (Model P7-17, S#1501699/2014). The employer did not ensure that employees implemented energy control application steps or equipment protective measure prior to allowing them to clean adhesive from a fixed base roll in the machine's Die Station section. The machine was not deenergized prior to the employee engaging in the servicing/maintenance activity.  As a result, the applicable energy control elements, involving preparation for shutdown [1910.147(d)(1)], machine shutdown [1910.147(d)(2)], machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.

29 CFR  1910.147(f)(3)(i):  When servicing and/or maintenance is performed by a crew, craft, department or other group, they shall utilize a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device:   At a facility located at 950 Breezewood Lane in Neenah, WI; Press Operator employees conducting preventative maintenance and cleaning on Press 1, identified as the Mark Andy P7 Printing Press (Model P7-17, S#1501699/2014), did not utilize a procedure which affords all employees working on the equipment a level of protection equivalent to that provided by the implementation of a personal lockout device. Press Operator employees did not affix their own lockout devices to the press' power disconnects while Maintenance employees conducted preventative maintenance operations on the same equipment.

Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.:  At a facility located at 950 Breezewood Lane in Neenah, WI; one or more methods of machine guarding were not provided to protect Press Operator employees from hazards such as the ingoing nip points at the Die Stations section of Press 1, identified as the Mark Andy P7 Printing Press (Model P7-17, S#1501699/2014). The ingoing nip points were present where the fixed base rolls met the anvil rolls, and where the anvil rolls met the die rolls.

The guard shall be such that it does not offer an accident hazard in itself:  At a facility located at 950 Breezewood Lane in Neenah, WI; the employer did not affix guards to Press 1, identified as the Mark Andy P7 Printing Press (Model P7-17, S#1501699/2014), at the Dies Station's north end where ingoing nip points from guide/driver rolls were present. The guards were equipped with handles and were held in place on the press via magnets.
