29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a. On the roof of a temporary residential new construction worksite, at 1652 Fruit Cove Woods Drive, Saint Johns, FL 32259: On or about September 19, 2022, the employer exposed five employees to eye injury hazards, in that eye protection was not utilized by employees who were operating Metabo nail guns to nail down roof shingles.

29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   a. On a 6:12 pitched roof of a temporary residential new construction worksite, at 1652 Fruit Cove Woods Drive, Saint Johns, Florida 32259:  On or about September 19, 2022, the employer exposed five employees to fall hazards, in that, fall protection was not utilized by employees installing roof shingles.  The employees were exposed to the hazard of falls ranging from 10 feet to 21 feet 7 inches to the ground.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  a. On the backside of the home, at temporary residential new construction worksite, at 1652 Fruit Cove Woods Drive, Saint Johns, FL 32259: On or about September 19, 2022, the employer exposed employees to fall hazards, in that the 12-foot aluminum extension ladder was only raised about 6 inches above the roof.
